The Breast Health Patient Navigator (LPN, RN) serves as a liaison
between the patient, Radiologist and Surgeons. - A central resource
to patient families and staff to ensure exemplary service providing
efficient, timely and coordinated guidance and support throughout
the care continuum (screening, diagnosis, treatment and
support/education services). - The Breast Health Patient Navigator
partners with patients and providers to offer a personalized,
individualized approach to women's health care including the
screening and diagnosis of malignant and benign breast disease.
Responsibilities will include but will not be limited to:
- - - - Provides patient education and support on pre and post
procedure instructions for Stereotactic biopsy, Ultrasound guided
biopsy and MRI guided Breast Biopsy, reinforcing information about
diagnosis, treatment plans and available resources.
- - - - Initiates communication with patients that are given the
recommendation for a breast biopsy. - Assist/direct the patients
with scheduling and screening for the biopsy. Assist with any
questions and/or concerns. Confirm with referring offices that
patient abnormal biopsy results were received.
- - - - Communicate and collaborate with the patient's referring
provider to improve timeliness of care and patient
satisfaction.
- - - - Create and maintain statistical analysis programs across
all breast imaging modalities
- - - - Work in collaboration with the imaging department to assist
in preparing case presentations for all cancer conferences related
to women's health.Qualifications Education:
- - - - Graduate of an accredited Licensed Practical Nursing or
Registered Nursing program; Bachelor's preferred
Experience:
- - - - 3+ years' experience, preferably in Women's Health
including breast imaging, breast disease, and/or gynecologic
disease.
License and Certification:
- - - - ---Current Licensed Practical Nurse or Registered Nurse in
the State of Connecticut.
